What information do we collect?

We collect anonymous clinical data that cannot be associated with any individual and does not constitute Personally Identifiable Information when you fill out any forms or provide other information within our web applications for processing clinical data. As described in greater detail in our Terms of Use, we request and you must agree not to enter any Personally Identifiable Information about any individual into the Curbside Consultant Mobile Application (APP) including any names, identification numbers, driver's license numbers, birthdays, or birthplaces.
